| All services & products are payable
on the day of the service unless special arrangements
have been made and approved by the Doctor and Patient
Accounts Manager. All nutritional products,
orthopedic supports, spinal and leg pillows, as well
as foot orthotics are taxable items that must be paid
the same day that the order is placed. ALL PRODUCTS ARE NON-REFUNDABLE DUE TO FEDERAL
HEALTH AND SAFETY STANDARDS.
